The Genetic Counseling Assistant will:
The Patient Navigator will provide case management services to our high-risk pregnant patients; this
person will serve as the main point of contact for patients. The Patient Navigator develops strong
relationships with service providers in order to help patients remain engaged in their medical care. The Patient Navigator is responsible for removing any barriers in the patients medical care such as identifying critical resources, assisting in the navigation throughout the health care system, and promoting key health. They will work closely with the Care Team; of which may include physicians, nurses, and other clinical staff to support positive health outcomes.
Requirements: - Minimum of two (2) years of medical office experience, fluency in medical terminology, and
proficiency in healthcare data entry are strongly preferred.
- Excellent organizational, planning, interpersonal, and time management skills. Able to manage
multiple projects. Strong follow-up and communication skills are necessary.
- Requires attention to detail and computer accuracy for data entry, patient scheduling, and
proficiency in using primary office equipment.
- Knowledge of Microsoft Office (Outlook, Excel, and Word)
- Bi-lingual (fluent in Spanish) strongly preferred.
